If you live within our practice area you are welcome to register with us. You can do this online, or by visiting us at the surgery, where our care navigation staff will be happy to guide you through the procedure.
You do not need proof of ID to register with a GP, but it might help if you have one or more of the following:
- Passport
- Birth certificate
- HC2 certificate
- Rough sleepers identity badge
If you’re a rough sleeper you can give a temporary address, such as a friend’s address, a day centre or the GP surgery.
If you need help registering or filling in forms, call the GP surgery and we will be happy to help.
You could also ask for help from:
- Local organisations – for example if you’re homeless you could ask a centre that supports homeless people
- Citizens Advice
- Your local Healthwatch
To register you will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information about you and your conditions. Medical treatment is available from the date of registration completion, which is usually 2-3 working days.

If you are unable to print the forms yourself, then we have registration packs available on reception. Please visit the surgery, and we can provide you with the relevant forms.
