
If you live within our practice area you are welcome to register with us. You can do this online via the link below.
You do not need proof of ID to register with a GP, but it might help if you have one or more of the following:
- Passport
- Birth certificate
- HC2 certificate
- Rough sleepers identity badge
If you’re a rough sleeper you can give a temporary address, such as a friend’s address, a day centre or the GP surgery.
If you need help registering please call the GP surgery and we will be happy to help.
You could also ask for help from:
- Local organisations – for example if you’re homeless you could ask a centre that supports homeless people
- Citizens Advice
- Your local Healthwatch
Medical treatment is available from the date of registration completion, which is usually 2-3 working days.

Once you have submitted this form, there is no need to contact the surgery. After your request has been processed, we will send the login details for your account to the email address you provided. You will not need to visit the surgery or take any further action to complete the setup.
