Our care navigators have the skills and knowledge to assist with signposting and support. They are vital to the smooth running of the practice.
The role of our Care Navigators has changed since moving to a total triage system. The team work closely with our triage doctors to provide outcomes to patients who have completed an eConsult, by allocating the queries to the triage doctor and contacting patients who need to be booked for an appointment.
For those who are unable to complete an eConsult online, the care navigators are here to help, either over the telephone, or in person.
To enable doctors to understand symptoms and specific needs the care navigation team gather data. We understand that the questions can sometimes appear long winded. However, they have been written by clinicians and are essential for us to access your needs correctly.
The team also help with queries, registrations and booking routine nurse appointments. They work tirelessly to answer calls and provide our front of house assistance.
