USEFUL LINKS FOR SELF-CARE

My mhealth have developed online self-management support for patients with COPD, Diabetes and Asthma and heart conditions  which we can now provide through our practice.

The apps are easy to use and can be accessed on almost any device that connects to the internet, including smart-phones, laptops, tablets, and even smart TVs.

We strongly encourage you to take full advantage of this opportunity to take control and improve your management of your condition.

If you would like access please Call 07873838142 and the Digital Health Adviser Bex from my mhealth will then register you on the App. Once they have registered you, you will receive an email from them with instructions on how to set up your account.

For more information and to see how it can work for you please see the website www.mymhealth.com

The Myheart App is for people with Angina, Heart Failure, recent heart attack or Acute coronary syndrome, Heart surgery or angioplasty
